The information that you must submit to CECBEMS after an activity has been held are listed in your accreditation documentation. You will also find the specific data pieces and formats in the Providers Manual.
After an activity has been completed, you must submit the following information to CECBEMS for each student completing the activity:
1. A list of participant names with the following required demographic information:
Email address (optional)
Student's license or certification type
State of licensure or certification
Expiration date of license or certificate
Date the activity (course) was completed
Your CECBEMS-assigned provider number
The student's city of residence
The student last name
The student's first name
The student's middle name (optional)
The student's street address (optional)
The student's NREMT reregistration date (if student is NREMT)
The student's state of residence (optional)
The student's zip code (optional)
The student's NREMT registration number (if student is NREMT)
The student's phone number (optional)
The CECBEMS-assigned activity (course) number
The number of CE units earned
The type of CE units earned
2. A summary of participant evaluations. (Please note that you should send only a summary of the data collected, not the individual participant evaluation sheets.)
